Medical - Registered Nurse / Registered Psychiatric NurseReporting to the DAL Supervisor, the Community Care Coordinator (CCC)-CM functions as case manager coordinating and integrating services and care for clients who reside in contracted assisted
living sites in the Calgary zone. The case manager has key roles and functions which include intake/screening and client engagement, assessment, care implementation and coordination of service
delivery, monitoring and evaluating care and outcomes, formal reassessment and disengagement. Case managers are experienced health professionals who are accountable and responsible for facilitating
the achievement of client based goals and applicable standards within a defined context. In order to achieve these client centered outcomes, it is essential that the Case Manager provide leadership
to the site based nursing team through excellent communication, leadership, interpersonal and conflict resolution skills. The case manager also ensures collaboration occurs with the site nursing
team, the family and /or the client. As part of the case management process, the Case Manager functions as a leader, advocate, educator, and steward of resources.

Rates of Pay: Minimum: 32.34 per hour Maximum: 42.45 per hour
Qualifications:
- Completion of a post secondary university degree or diploma program in Nursing.
- Three years of recent nursing experience including geriatric experience (obtained within the last five years); in the absence of recent experience, completion of a refresher program within the last
two years may be considered. Previous community related experience a definite asset.
- Active registration with CARNA.
- Geriatric certification an asset.
- Excellent interpersonal, leadership, conflict resolution, and communication skills.
- Excellent organizational and time management skills - be able to efficiently organize many details in a time efficient manner.
- Ability to monitor financial accountabilities related to care delivery.
- Proven critical thinking and decision making skills.
- Possession of a valid driver's license and access to a motor vehicle in good repair for work related travel.
- Physically able to perform all assigned tasks.
- Ability to perform all duties in accordance with related legislation.
- Demonstrated ability to provide comprehensive assessment skills of individuals within the context of their physical and social environment.
