primary care physicians using an integrated care model. Consultants identify, assesses, triage, and manage patients with medical and behavioral health problems in the primary care setting. The
consultant will develop specific behavioral change plans for patients and behavioral health protocols for target populations. Successful candidates will be provided with specific training in the
integrated care model, appropriate supervision, and ongoing consultation and support as required.

Rates of Pay: Minimum: 36.48 per hour Maximum: 48.48 per hour
Qualifications:
- Minimum of Masters Degree required from Social Work, Psychology, or Nursing.
- Must be eligible for professional registration.
- Minimum of four years demonstrated experience in assessment and treatment of mental health disorders.
- Training and experience in evidence-based interventions and basic health psychology.
- Ability to work within a primary care behavioural health model.
- Knowledge of and ability to apply population-based care principles.
- Knowledge of basic medications.
- Sound judgment and strong interpersonal skills.
- Self-directed, flexible, and displays initiative in new and evolving role.
- Ability to develop collaborative working relationships with primary care professionals.
- Ability to provide very accessible and responsive consultation.
- Strong time management and organizational skills responsive to primary care settings needs.
- Comfortable presenting to groups of people.
- Program and service development skills.
- Speaking and hearing sufficient to communicate effectively by phone or in person, at normal volumes.
- Vision adequate to read correspondence, computer screen, forms, etc.; good manual dexterity.
- Possession of valid Driver s License and access to a vehicle is a requirement of the position and condition of employment as travel is required within the Region.
- Assist primary health care providers in assessing recognizing and treating mental health disorders and psychosocial problems.
- Assess the clinical status of patients referred by primary care providers through brief consultative contacts.
- Work with primary care team to treat and manage care for patients with chronic emotional and/or health problems efficiently and effectively.
- Work with primary care provider to refer cases to mental health specialists when appropriate.
- Assist in the detection of at risk patients and development of plans to prevent further psychological or physical deterioration.
- Assist in preventing relapse or morbidity in conditions that tend to recur over time.
- Travel in northeast quadrant of the City of Calgary.
