Reporting to an Area Manager, the Community Care Coordinator, screens and assesses the needs of individuals and their families who cannot function
independently in the community; by mobilizing and managing health and resources required. In addition, monitors resources to ensure they improve or maintain the person's and the families' abilities
to function safely and idependently in the community.

Employee Class: Temporary Part Time
Rates of Pay: Minimum: 32.34 per hour   Maximum: 42.45 per hour
Qualifications:
- Completion of a post secondary diploma or university degree in Nursing.
- Current registration with CARNA.
- Minimum two years of recent experience, previous community experience in geriatric an asset.
- Gerontology Certificate an asset.
- Excellent interpersonal, communication and time management skills and is a team player in an interdisciplinary team.
- Proven critical thinking, decision making and assessment skills.
- Ability to monitor financial accountabilities.
- Physically able to perform all assigned tasks.
- A valid Alberta driver's license/access to a vehicle.
- Ability to perform all duties in accordance with related legislation.
- Basic computer skills are an asset
