Registered Nurse

Reporting to the Care Manager, the Community Care Coordinator (CCC) requires strong interpersonal communication and inter-disciplinary team player
skills.  Responsibilities include: assessment of needs, planning, delivering and coordination of nursing care, communication  with resources and services within professional practice standards,
establishing goals for client delivery, and evaluating outcomes for the delivery of holistic, comprehensive care to meet the needs of individuals and families in the community.

Rates of Pay: Minimum: 32.34 per hour   Maximum: 42.45 per hour
Qualifications:
- Graduate of an accredited diploma or degree program in Nursing.
- Active CARNA registration.
- Two years of recent experience (obtained within the last 5 years).
- Possesses a valid Alberta Driver's license and access to a motor vehicle in good repair for work-related travel.
- Self-directed and demonstrates the ability to work independently using sound professional judgement.
- Proven critical thinking and decision making skills.
- Excellent organizational and time-management skills to be able to prioritize and efficiently organize details in a rapidly changing environment.
- Strong assessment skills necessary.
- Geriatric certification and asset.
- Thorough knowledge of Strathmore and area community and health resources and asset.
- Strong interpersonal skills for making and maintaining community partnerships.
- CPR certification required.
- Basic computer skills an asset.
