Administrative - Entry LevelThe Calgary Foothills Primary Care Network (CFPCN) is a group of family physicians in North West Calgary and Cochrane collaborating with Alberta Health Services to deliver the best possible primary care. CFPCN brings together physicians and other primary health care providers to achieve efficiencies, assess and meet patient needs and cooperate on special programs that serve the Calgary Foothills area. The network operates in partnership with Alberta Health and Wellness, the Alberta Health Services and the Alberta Medical Association.
CFPCN has established a team environment where individuals are encouraged to take initiative, be creative and contribute to ongoing decision-making. The PCN Liaison / Project Manager is an integral member of the Primary Care Network Team. The liaison / project manager provides information to CFPCN physicians and assists primary care providers to develop effective health care teams. The liaison / Project Manager will develop and manage projects in CFPCN program areas.
Key Responsibilities:
The percentage of time spent in each category may change dependent upon the needs of the program. Key aspects of the position may include, but are not limited to:
LIAISON (0.5 FTE)
Provide a first point of contact for CFPCN physicians
Direct visits to approximately 50 CFPCN physician offices
Facilitate the introduction and uptake of PCN and CHR resources into physicians' offices
Collect and distribute physician feedback and suggestions
Provide external leadership to physicians interested in developing office efficiencies, protocols and optimizing the medical home
PROJECT MANAGER (0.5 FTE)
Works with the Medical Lead to develop CFPCN projects in program areas such as mental health, population health or chronic disease prevention and management
Ensure the provision of a high quality service; identifies new service requirements and deficiencies in current services, and develops plans to address requirements
Participates in the development of program outcomes
Participates in the measurement of program outcomes
Qualifications:
Degree in health care field; combination of experience and education will be considered.
Experience:
Experience in primary care considered an asset

Excellent communication skills

Ability to work independently
Ability to thrive in a dynamic, evolving environment
Ability to design, create and implement programs of service design
Group facilitation skills will be considered an asset
Good computer skills are required
