Reporting to an Area Manager, the Community Care Coordinator screens and assesses the needs of individuals and their families who cannot function
independently in the community; by mobilizing and managing health and social resources required. These clients all live within the urban area.

Rates of Pay: Minimum: 32.34 per hour   Maximum: 42.45 per hour
Qualifications:
- Completion of a post secondary, diploma or university degree program in Nursing.
- Two years or recent experience (obtained within the last five years); in the absence of recent experience, completion of a refresher program within the last two years may be considered. Previous
community related experience a definite asset.
- Active registration with the CARNA.
- IV certification preferred.
- Excellent interpersonal and communication skills.
- Excellent organizational and time management skills.
- Be able to efficiently organize many details in a time efficient manner.
- Excellent assessment skills including over the phone assessments.
- Ability to monitor financial accountabilities.
- Proven critical thinking and decision making skills.
- Possession of a valid driver's license and access to a motor vehicle in good repair for work related travel.
- Physically able to perform all assigned tasks
