Registered Nurse

Reporting to the Area Manager and the Onsite Care Manager, the Community Care Coordinator screens and assesses the needs of individuals and their
families who cannot function independently in the community. The incumbent provides case management, direct nursing care and is able to work in a multidisciplinary team to coordinate and provide
comprehensive care and health resource management thereby enabling patients and their families to function in the community

Rates of Pay: Minimum: 32.34 per hour   Maximum: 42.45 per hour
Qualifications:
- Completion of a post secondary diploma - university degree in nursing is preferred.
- Geriatric experience/certification is an asset.
- Recent Home Care related experience is preferred.
- Active registration with CARNA.
- Excellent interpersonal and communication skills.
- Excellent organizational and time management skills- be able to efficiently organize many details in a time efficient manner.
- Ability to monitor financial accountability and proven assessment skills.
- Proven critical thinking and decision making skills.
- Possession of a valid drivers' license and access to a motor vehicle in good repair for work related travel.
- Physically able to perform all assigned tasks.
- Able to perform all duties in accordance with related legislation.
Current CPR required
Experience with PARIS; RAI, and AADL authorization is preferred
