Reporting to an Area Manager the Community Care Coordinator screens and assesses the needs of individuals and their families who cannot function
independently in the community; by mobilizing and managing health and social resources required. In addition, monitors resources to ensure they improve or maintain the person's and families'
abilities to function.

Rates of Pay: Minimum: 32.34 per hour   Maximum: 42.45 per hour
Qualifications:
- Completion of a post secondary, diploma or university degree in nursing.
- Two years of recent experience obtained within the last five years.
- Active registration with the CARNA.
- Home Care experience preferred.
- Excellent interpersonal and communication skills.
- Excellent organizational and time management skills.
- Ability to monitor financial accountabilities.
- Demonstrated critical thinking and decision making skills.
- Gerontology certificate an asset.
- Demonstrated assessment skills.
- Physically able to perform all assigned tasks.
- Demonstrates ability to work independently in unique environments.
- Ability to perform all duties in accordance with related legislation.
- Possession of a valid driver's license and access to a motor vehicle in good repair for work related travel
