The Transition Service Coordinator screens, assesses, coordinates and evaluates the needs of patients and families who cannot function independently in
the community.  The Transition Service Coordinator collaborates and assists the units in expediting discharges for individuals who require community services.  This may include arranging home care
support and linking patients to the appropriate community agencies

Employee Class: Regular Part Time
Rates of Pay: Minimum: 32.34 per hour   Maximum: 42.45 per hour
Qualifications:
- Nursing degree preferred
- Three years of recent experience in a community setting or acute care setting
- One year recent Transition Service experience required
- Community related experience preferred
- Current CPR Certification
- Active registration with CARNA
- Excellent interpersonal, communication and organization skills
- Ability to monitor financial accountabilities
- Proven critical thinking, decision-making skills and assessment skills
- Physically able to perform all assigned tasks
- Demonstrated ability to perform all duties in accordance with related legislation
- Demonstrated familiarity with computer programs required
