Primary Care Networks (PCN) are a made-in-Alberta approach to ensure that people receive the right care from the right health professional at the right time, every time. Mosaic Primary Care Network is located in the NE of Calgary. More than half of the 350,000 people living in this catchment area are attached to one of the 175 MPCN physicians. This population has a higher than provincial or city average of: unemployment, low income, refugees, immigration, hypertension, uncontrolled diabetes, and a variety of other chronic conditions. NE Calgarians are more likely to have learned English as a second language. Improving their health is a significant challenge.
POSITION SUMMARY
Reporting to the Director Clinical Services, the Team Lead- Chronic Pain provides comprehensive nursing services to the patient population within the Calgary Mosaic Primary Care Network. This 1.0 FTE position is self-directed and is responsible for assessing, managing, and supporting patients, while providing appropriate documentation. This position works collaboratively with the Physician Lead Chronic Pain and the multi-disciplinary team to develop and implement care plans, evaluate outcomes and provide follow-up with the patient. The Team Lead- Chronic Pain works to full scope of practice as a member/leader of a health care team.
The Team Lead- Chronic Pain works to ensure a high quality of evidence-based health care to the practice population and in accordance to the Standards defined by the College and Association of Registered Nurses of Alberta Standards of Practice.
The Team Lead- Chronic Pain is responsible for providing leadership for the multi-disciplinary team and operations at the Mosaic Chronic Pain Clinic.
KEY RESPONSIBILITIES AND ACTIVITIES
The Team Lead- Chronic Pain will utilize the EMR system or paper chart in maintaining accurate and timely records, documenting all phases of client care.
1.0 Clinical Role
Provides/coordinates care/services with other team members and community resources, appropriate to the situation, and in accordance with Mosaic PCN policies, procedures and established standards.
Receives patient referrals from MPCN physician members and coordinates the health services required by each individual patient based on developed nursing knowledge and skills.
According to practice protocols, conducts assessments, initiates and implements management strategies and provides appropriate evaluation, follow-up and modifications to care plans.
Makes professional collaborative decisions in managing patients and offering a complementary source of care to that offered by multi-disciplinary practitioners.
Assists in the implementation of strategies with the physician Lead- Chronic Pain for improving the quality and provision of care for the practice population.
Works in conjunction with the Physician Lead Chronic Pain as appropriate to assess the needs of patients.
Assists in the delivery of disease management and health promotion programs and provides appropriate care and treatment according to practice policy and protocols.
2.0 Leadership
In consultation with the Director of Clinical Services leads the development of the Chronic Pain Program, objectives and systems and supports program measurements that support the accomplishment of MPCN strategic objectives.
Develops and maintains a strong team that is accountable for the management of and delivery of care for those patients with chronic pain and ensures the activities are consistent with the goals of MPCN.
Ensures a high quality of personnel through effective selection, promotion and retention programs.
Works collaboratively with the Director of Clinical Services and the Evaluation Coordinator in the planning, operation and evaluation of the Chronic Pain program.
SUPERVISION
The Team Lead- Chronic Pain reports to and is supervised by the Director of Clinical Services.

QUALIFICATIONS

Education: A Baccalaureate degree in Nursing from an approved health care program with current CARNA registration is required, together with current CPR certification. Must have own vehicle and a valid drivers' license
